With more than 109,000 points of access nationwide, Superior Vision gives members the freedom to choose where they receive care, whether from local, independent optometrists or major retail optical chains.
In addition to affordable coverage for eye exams, glasses, and contacts, Superior Vision offers members discounts on vision correction surgery, including LASIK and PRK, through the QualSight LASIK network.
How to Find a Superior Vision Provider
Finding a participating provider is simple. Visit the Superior Vision Provider Locator to search by:

Provider or practice name
Network type or specialty
You’ll find a wide range of options, including private optometry practices like Reflect Vision Care as well as national retailers.

What If My Provider Isn’t In-Network?
You’re free to see any provider you prefer, but if your provider is out-of-network, you’ll typically need to pay upfront and submit a claim to Superior Vision for reimbursement.
The good news? Superior Vision’s out-of-network benefits still allow you to use your full frame, lens, or contact allowance, though reimbursement amounts may vary.

How Much Does Superior Vision Cost?
Superior Vision is available both through employers and as individual plans for those who are self-employed or not covered through work.
Individual plans start around $12.50 per month
Family plans are available for slightly higher monthly rates

Coverage typically begins on your selected start date
You can enroll online year-round and begin using your benefits as soon as your plan is active.

Eye Exams
Most plans include a comprehensive eye exam with a copay ranging from $0–$20.
Frames and Lenses
Frame allowance: typically $125–$200
Lenses covered in full: single vision, lined bifocal, and lined trifocal
Optional lens enhancements include:
Anti-glare coating
UV protection
Transitions or photochromic lenses
Premium progressive lenses
Members can also receive additional discounts on lens add-ons and second pair purchases.
Contact Lenses
Most Superior Vision plans include:
Contact lens fitting copay: typically $25–$50
Contact lens materials allowance: often up to $150
As with most vision plans, you’ll need to choose whether to apply your benefits toward glasses or contact lenses during the same benefit period.
Are Medically Necessary Contact Lenses Covered?
Yes, partially. Your frame or lens allowance may also be applied toward medically necessary contact lenses, such as scleral, hybrid, or rigid gas permeable (RGP) lenses for conditions like keratoconus or severe dry eye.
However, most plans do not cover the full cost of specialty contact lens fittings and materials. Our doctors can help determine your eligibility and out-of-pocket estimate based on your plan.
Does Superior Vision Cover Diabetic Eye Exams?
No. Superior Vision covers routine eye exams, but medical eye exams, including those for diabetes, glaucoma, or eye injuries, should be billed through your medical insurance.
If you have diabetes, annual comprehensive eye exams are essential for early detection of diabetic retinopathy, the leading cause of preventable blindness in adults. Our team will ensure your visit is billed under the correct insurance plan based on your needs.
Does Superior Vision Cover LASIK or PRK?
While Superior Vision does not directly pay for refractive surgery, members receive 15–50% discounts on LASIK and PRK procedures through the QualSight LASIK network.
